·         Consent to Treat and the Participant Code of Conduct ( or bring the forms completed – available at the URLs bellow)

·         A check for $100 payable to the Glades Youth Hockey organization.

·         USA Hockey Confirmation number for the 2012-2013 season; this is completed on-line.

o    Select ‘Ice Players & Coaches’

o    The online form walks you through the process

o    Make sure to print out the form with the player number, but if you have problems, make sure you write down the number.
